Legally Blind Registration Forms –
All Teachers

Each student on your caseload who is legally blind should be registered with the New York State Education Department. A student is considered legally blind if s/he has a measured visual acuity of 20/200 or less in the better eye with the best correction or a restricted visual field of 20 degrees or less. Use the student’s medical eye report (E12S) to determine whether a student meets the definition of legal blindness.

Complete all sections of the New York State Education Department Registration Form for each student who is classified as legally blind on your caseload. You only need to register a student once. If a student on your caseload has already been registered, you do not need to complete a new registration form. If you are unsure whether or not a student on your caseload has been registered, contact your EVS Supervisor. Send one registration form for each legally blind student to EVS, Attn: Jeannine Hobbes. Registration forms are due by Friday, December 1, 2017  and will be available on the EVS website.
